Try putting "custom_maps" higher up, and see if it works. I was having the same kind of problem, sometimes I put the dlcpack higher up and it worked. I don't know if it is the source of the problem, but after I increased the limit on my Heap Adjuster I didn't have this kind of problem anymore.
